Welcome to Living Water department forms

This site lets enrolled students and applicants submit official requests to university offices — CPT, withdrawals, refunds, registrar documents, and more.

You do not need a password to use the public forms. After you submit, use the Track request tab to follow your case, read office messages, upload extra files, and download approved documents.

1. Choose the right department tab

2. Verify your Populi email (enrolled students)

Most department forms require you to confirm the email address on your Populi student profile before the form opens. This keeps requests tied to your enrollment record.

  • Step 1 — Enter your Populi profile email and click Continue. The system checks that you are enrolled (or pending enrollment).
  • Step 2 — The form opens with your name, student ID, phone, and address filled in from Populi when available.
  • Fields filled from Populi cannot be edited — use Use a different email if you need to start over with another address.
  • Verification lasts about 2 hours in your browser session.

Note: Admission applications and Track request lookup do not use this Populi verification step.

3. Complete and submit your form

Read each section carefully. Attach supporting documents when the form asks for them (PDF or image files; size limits are shown on the form).

  • Click Submit once — a Please wait screen appears while your request is processed. Do not refresh or click Submit again.
  • You receive a confirmation email at the address on the form with a link to Track request.
  • The office receives a PDF copy of your submission and any uploads you included.

4. Track your request and talk to the office

Open the Track request tab and enter the same email you used on the form. You see every submission stored on this portal for that email.

  • Status shows Pending review, Approved, or Declined.
  • View & respond opens the conversation — read staff messages, send a reply, and attach up to 5 PDF or image files (8 MB each).
  • When a request is Approved, Your approved documents lists downloads such as the application PDF and, for CPT, Form I-20 when the office has sent it.

5. Available forms (direct links)

You can bookmark or share these links. Each form still requires Populi verification on step 1 when you are not already verified in this browser session.

6. New applicants (Admission tab)

If you are applying to the university, use the Admission tab to choose your application type and continue to the correct Populi application page.

  • Application fees are non-refundable. Read the LWU Application Guidelines before you continue.
  • Admission applications are completed in Populi — they are not listed on Track request on this site.
  • Questions about application types not listed on the page: admissions@livingwater.education

Quick tips

  • Use the email address you check regularly — Track request and office messages use that same address.
  • If you do not see a confirmation email, check spam and verify you entered the correct address on the form.
  • For urgent ISO or immigration questions, contact the office directly using the phone number on the form letterhead.
  • Staff sign-in on the sidebar is for university employees only — students never need that login.
